No SMART does not work with Raw Disk Mapping. BUT if you use ESXi pass though with VT-d it does work. Will be doing a video on this as well. Also this might get fixed in a future version of gnoBoot.
Ok. Thanks for the fast reply. The issue is that I have a HP N54L and it doesn't support VT-d. This is the only thing that prevents me to run DSM in ESXi. When I tested OMW(OpenMediaVault) SMART worked fine with RDM, so hopefully gnoBoot will figure this one out.

I don't think so. I've read it here that you need to enable/disable RDM filter to make it work. I just couldn't remember which thread it was.

How to check HDD SMART status:
1. Shut down Synology VM
2. Clear the RDMFilter flag on your ESXi server:
- Configuration - Software - Advanced Settings - RdmFilter.HbaIsShared=OFF - Click OK
3. Start Synology VM
4. SSH login
5. Check the first RDM drive in the array:
- smartctl -a /dev/sda

No, USB is used just for boot loading, rest of install and config resides on your HDDs.

From which version are you trying to update?
I suppose you're trying to apply over the 4.3 build 3810 my offline package of v4 update.
Try again my offline method but without connection to internet.

@muhh: it is not so hard to customize it yourself after a fresh install but:
... would be nice to have: NFS + CIFS + bootstratp + open-vm-tools + Update 4 included into a complete package - ESXi v1.2 maybe ?@ Trantor: as more and more resources are directed to DSM v5, maybe would be good to have a final complete release with DSM v4.3 for ESXi .

I had to wait for a few replication jobs but after restarting the VM still no SMART on my devices.
Can't remember what "smartctl -a /dev/sdc" says about Vendor, Product, Revision (VMware, Virtual Disk, 1.0 or something else) but it does not work.
Just thinking: I suppose that you have SMART option enabled in BIOS...
